To safeguard you and minimise the spread of COVID-19, Luke requests that you complete this form as a pledge of your circumstance for the day of your shoot. One form per client per booking. This information will help determine any precautionary measures required to protect all parties, based on NSW Health advice.

Are you fully vaccinated?

If YES, please present your COVID-19 vaccination certificate at your shoot. If NO, you must provide a negative COVID-19 test result within 48 hours prior to your session.
